HarmonyCares part of Iowa total care scam

A phishing scam report, targeting a consumer in Waukee, IA, USA - 50263.

Narrative evidence

What happened

Scammer called using a caller ID matching the name of my insurance company and asked for me by name. Scammer claimed to work in-partnership with the insurance company as part of HarmonyCares to offer in-home and telehealth services before reading out my full name and date of birth minus the year. Scammer repeatedly asked for the year of my birth becoming verbally agressive and agitated anytime something other than the year was stated. After 3-4 escalations I surrendered the year and the scammer read out my full home adress minus the zip code and repeated process listed prior. Then the scammer asked my if I would prefer one of a number of options (including in-home and telehealth) for treatment and claimed some form of technical error any time an option for other than telehealth was chosen and steered the conversation towards the telehealth option. The scammer then asked for a prefered date with 1 weeks time to schedule the telehealth appointment repeating the escalatory process when indiffrence was expressed. When the date was set the scammer claimed I would be seen by practioner Nicole Fulla(?) (full-uh) and said they send me a link to download an app for the apointment the day before the apointment was scheduled to occur. The called ended with the scammed asking if I had any questions and when I said no they volunteered the phone number 855-850-6248 for the pursposes of "contacting us".
